Out of Hours
The provider of out-of-hours care is named Primecare. In an emergency, out of telephone hours, please telephone the surgery number where your energency call will be diverted

Test Results
Patients are responsible for finding out results of their x-rays, blood tests and urine/stool samples. Please telephone for your results after 3.00pm.

Home Visits
Whenever posssible, please make requests for visits before 11.00am. Home visits are at the doctor's discretion.

Patient Registration

Patients are registered with the practice, not with an individual GP. Your medical card will be issued in the name of one of the doctors who will supervise your care. However, you can at any time express a preference for a practitioner, for either all of your medical needs or on a case-by-case basis. This preference will be recorded in your medical notes. Whilst the practice will always try to comply with your requests on all occasions, it may not always be possible.

The Partnership of Drs Cooper, McAllister, Dollery, Plate, Stacey, Crane, Hunt & Graham is a full equal partnership with no restriction or limitations. The partnership is contracted to the NHS to provide general medical services to patients registered with our practice and temporary residents within our practice boundary (as agreed with the NHS). We also provide immediately necessary treatment for any person not registered with the practice who has a medical emergency within our practice boundary.

All patients registering at our surgery will be asked to complete a new patient questionnaire. Children under five years of age will be assessed by our health visitors, which will enable you to discuss any questions regarding your child's health, immunisations, development or behaviour.


Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
