New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Patients are registered with the practice, not with an individual GP. Your medical card will be issued in the name of one of the doctors who will supervise your care. However, you can at any time express a preference for a practitioner, for either all of your medical needs or on a case-by-case basis. This preference will be recorded in your medical notes. Whilst the practice will always try to comply with your requests on all occasions, it may not always be possible.

Registration Process
To register with the Surgery you will need to complete two forms
- Purple GMS1

- New patient registration form
You will also need to provide two forms of ID (16+only)
- Photo e.g. passport or driving licence
- Formal proof of address e.g. utility bill or bank statement (no older than 1 month)
Once the forms have been completed, ID seen and your address confirmed as being within our catchment area then the registration can be completed by our administration team.
Please allow 2-3 working days after handing in your documentation before trying to book an appointment.
Repeat Medication
If you are on any repeat medication, please ensure that you provide us with a copy of your prescription at the time of handing in your documentation.
Alternatively you may print off these registration forms, complete and bring them in with you on your first visit to the practice.
